Hazardous Waste Arator Program Inspection Report date arch, 19,2008 Pep Boys Store #711, 4987 West Lane, Stockton, CA 2 of 3 All new employees are required to take the "New Employee Orientation" via the store Intranet which encompasses the following subjects: (1) Environmental compliance; (2) DOT general awareness; (3) Hazardous communication procedures; (4) Safety. Four separate videos also accompany the training section and the employee must complete a quiz at the end of the course. Once the employee "passes" each section, the program automatic logs the employee as having completed the orientation. In addition all employees are required to take an annual environmental refresher. Attachment 4 is the paper copy of the "New Employee Orientation" training module. Due to the size of the entire training module, I have only included the Environmental compliance, DOT general awareness, Hazardous communication procedures and Safety sections. ■ Attachment 5 is the hard copy of the "Environmental Update Refresher". This refresher training is taken annually and it also requires a quiz to be completed at the end of the course. • Attachment 6 details all the employees at this location with the date the "New Employee Orientation" and "Environmental Update Refresher" was completed. #64- Failed to retain on-site required certification for HW tanks. (66265.192 CCR) We are certain that the used oil tank was inspected by the local Fire Department prior to its installment; unfortunately we have been unable to find any documentation verifying this. It has come to our attention that we may qualify to file for exemption under 22CCR 66265.1926). We are investigating this option. At this time I would like to respectfully ask for an extension to bring this violation into compliance. The goal is to have a definitive answer by May 6`h. Please let me know if this will be an issue. #69- failed to conduct dally inspections of HW tank. (66265.1959(a) CCR) The store maintains a daily reading log of the used oil tank. The used oil tank gauge is read twice daily at opening and closing time. Attachment 7 is a copy of the most current daily reading log for your review.
