Instructions: <br />How to Use Hazardous Waste Accumulation Labels <br />After your waste antifreeze, waste water or waste oil (the type of waste varies from store to store) has <br />been drained from its container, a new label must be adhered to the container (either drums or tanks). <br />A NEW label must be affixed to containers each time the waste has been removed <br />• Complete the generator information (your store name and address) <br />• Enter your EPA ID Number. This number is specific to your store and should be listed on previous <br />waste manifests. If you cannot locate your EPA ID No., please contact PBY Environmental Dept. <br />• Fill in the accumulation Start Date and Contents information when the liquid level in the container is <br />half way full. <br />• The state waste code number for each waste is as follows: <br />waste oil — 221 <br />antifreeze — 135 <br />wastewater - 221 <br />• By law, a hazardous waste cannot be on a generators site for more than 90 days from the <br />accumulation start date. Therefore, you must monitor how long your waste has been on site. <br />• These accumulation labels will provide various officials with the information that they will require <br />pertaining to how long the waste has been on site, in the event of an inspection <br />If you have any questions, please contact the Environmental Department at (215) 430-9645. When you <br />run low on your label supply, please send a "forms request sheet" to the Forms Department. Indicate that <br />you need more Hazardous Waste Accumulation Labels. <br />If you are nearing the 90 -day storage limit, please contact the Pep Boys waste vendor at (800) 334- <br />3794 immediately. <br />Please keep this document and all labels in your Environmental File. <br />
