Name: Identification of who is generating the waste <br />b. EPA ID No.: Specific identification of the generator <br />c. Accumulation State Date: The date that waste is first introduced to the drum (90 days) <br />d. Contents Composition: What is the waste in the drum; i.e. Waste oil, waste coolant <br />e. Physical State: Is it a liquid, solid, aerosol <br />f.
