06/18/03 Faxed copy of blank financial assurance. To DPI JJ <br /> 06/16/03 Received a call from Doug Baker. He asked questions about issues <br /> and expressed a willingness to meet all inspection requirements. He <br /> did however state that he had a full time staff person working on the <br /> hazardous waste disposal. He stated that this person was about <br /> 2/3rds through the waste. He asked if there was any way that the <br /> time could be extended for this issue. I told him that this was an <br /> important issue that needed to be abated in an expeditious manor. I <br /> asked him when he expected this issue to be abated and he stated <br /> that his projections were July 11`" 2003. I explained-that I was <br /> e as is extensio , <br /> < I''1 11111!acknowledged this new time frame as an act of good faith. I asked <br /> for Pic ture-s'1:11:: 1 11rove e of the empty containers on July 11`h. I <br /> ,uns <br /> also informed him that there was not a record of any PEA, or any <br /> documentation from DTSC for an order to treat Cyanide. I <br /> informed him that the contact for this order was a woman by the <br /> name of Ausha. Doug stated that he would call Leo Valdez and get <br /> the contact information from Leo. JJ <br /> 05/20/03 oday I went to Delto Plating and n��1, _ �_[�pies of both <br /> hazardous waste fitting reports for Doug Baker. I <br /> discussed the finding with him and went through the photos one by <br /> one to enshure he understood the problems in them. I also discussed <br /> the time limitations and expectations for compliance of specific <br /> issues. At the end of the inspection review Doug was encouraged to <br /> ask an questions. Another card was left on site. JJ <br /> 05/15/03 While discussing this site to Doug Wilson-Supervising REHS it was <br /> brought to my attention that an OSHA referal was in order for poor <br /> housekeeping,possible cyanide gas issues and possible confined <br /> space issues. The referal was made today. JJ <br /> 05/13/03 Today Leo Valdez and I went out to the site to conduct a paper See next <br /> inspection. At this time I <br /> left matted copies of photographs along with commentary with <br /> Doug Baker. cont) <br /> fr <br /> 05/13 <br /> JJ <br /> 05/06/03 Today I conducted part of the inspection(Tiered Permiting) at this <br /> site. The inspection was conducted by Kasey Foley-Supervising <br /> REHS,Leo Valdez-Hazardous substances scientist DTSC. Mike <br /> Tucker took us through the facility after placing a call to his <br /> supervisor Doug Baker. Doug Baker Granted approval for the <br /> inspection. At the time of the inspection we were unable to look <br /> through the documentation because the person in charge of filing <br /> these documents was at home recovering from surgery. We were <br /> told that if we came back next week Doug Baker would be able to <br />
