ADDENDUM <br /> September 10, 1991 <br /> I phoned Doug Baker to confirm handling of cyanide at present. <br /> Mr. Baker stated the cyanide was separated from the <br /> hydrochloric and nitric acids. Mr. Baker also stated the <br /> drums were covered and labeled with a "Material Safety Data <br /> Sheet" (a label Mr. Baker created with paper that states <br /> accumulation start date and sludge analysis) . Mr. Baker said <br /> he also posted emergency plan and exits. When asked to verify <br /> the date of the Biennial Report, Mr. Baker stated he had <br /> submitted one on August 12, 1988 and another report on <br /> September 5, 1991. <br />
