All <br />samples will be kept in coolers packed in blue ice. <br />Sample Shipment and Chain of Custody <br />All samples will be shipped directly to the laboratory from the site in sealed sample <br />storage coolers. Chain of custody will begin at the time of sample collection by placing the <br />packaged and labeled samples in the cooler and covering them with blue ice. At the end of the <br />day all sample documentation and chain -of -custody forms will be completed. All required <br />documentation will be sealed in a resealable plastic bag and taped inside the lid of the cooler. <br />Coolers will be sealed with tamper -proof custody seals that will warn against tampering. <br />The following procedures will be followed when completing the chain -of -custody form: <br />Proiect Name Enter the project name. <br />Pro iect No. Enter the complete project number. <br />Samplers Enter the signature and print the names of people who participated in <br />collecting the samples and who should be contacted if questions arise during <br />sample log -in. If the field sample custodian is not listed as a sampler, receipt <br />documentation should be indicated. <br />Sample No. Enter I.D. number. <br />Date and Time Enter the date and time of sample collection. <br />Sample <br />Location Enter site name. <br />Parameters Indicate the analyses requested for each sample listed. Identify analysis <br />method if not indicated on the form. <br />No. of <br />Containers Enter the total number of sample containers for a given location. <br />Remarks Enter any remarks related to sample identification, chain of custody, or field <br />observations. <br />17 <br />
