QC NARRATIVE(S) <br />Subject: General Comments: <br />PAGE 1 <br />No Interruption, No Precipitation <br />Temperature Min 7OF Max 9OF <br />Geofon crew arrived on 0600 hrs Concrete removal was completed. Allegre <br />Truck arrived at 0700 hrs. A total of 7 loads of concrete debris and AC debris <br />were loaded and off -hauled. Geofon crew began exposing the fuel lines and vent <br />lines. We ecountered a little difficulty removing the nuts and bolts from the <br />manhole covers. A compressor and spud breaker had to be rented. Don <br />Valentine arrived on site and delivered generator information for Mani Pests_ <br />Dan Beever also arrived on site to deliver generator information. Both the COR <br />and base representatives were made aware of the UST pull schedule. All of the <br />piping was triple rinsed into the USTs. No leakage occurred_ PID readings of <br />the excavated pea gravel were less than 3 ppm. The excavated pea gravel was <br />stockpiled on visqueen approximately 30 feet west of the excavation_ <br />Subject: Safety Inspections/Meetings: <br />All areas were inspected for safety issues. It was noticed that the heavy <br />equipment did not have fire extinguishers. The rental company was called, and <br />fire extinguishers were furnished within the hour. Safety issues including <br />safety around small machines were reviewed. Workers were reminded to wear <br />their safety glasses and ear plugs. Safety for other pedestrians was also <br />mentioned. Traffic control issues were reviewed. <br />8.
