Roth Road, Lathrop <br />The proposed application for permit to remove two underground storage tanks at the above - <br />referenced location, bas been approved with the following conditions: <br />1. All inspections must be scheduled at least 48 hours in advance. <br />2. Call USA Dig prior to any excavation. <br />3. Underground Storage Tank Disposition Tracking Record must be submitted within 30 days <br />of tank removal. <br />4. The Backfill Excavation Certificate must be submitted within 30 days of the tank removal <br />with verification of backfill material. <br />5. The Tank Closure Certification form must be submitted within 30 days of the tank removal. <br />6. Sample analysis must be conducted in accordance with the diesel fuel specifications on page <br />8 of the permit application. <br />7. The amount of samples taken Will be based on the specifications on page 7 of the permit <br />application. <br />8. Analytical results must be submitted directly from the lab to PHS-EHD within 30 days of the <br />tank removal. <br />9.
