In addition, Shell's UGST Financial <br />Responsibility Document is enclosed. <br />The source of discharge was due to corrosion of the Regular Unleaded tank <br />#4, which was part of two 5,000 gallon manifolded tank configuration. <br />Below is an outline of the sequence of events: <br />12/7/94 - Shell notified by service station operator (Dealer) of a <br />suspected loss of product. <br />if - Shell's Sales Representative personally audited operators <br />inventory records. <br />if - RU tank operation was stopped. <br />of - San Joaquin Env. Health, Mike Kitt notified of a suspected loss <br />of product. 4:02 p.m. <br />of - RU tanks drained and Shell Contractor, NDE, performed an <br />ullage test. <br />12/8/94 - OES was notified and assigned OES# 005581. <br />- Ongoing evaluation of effective corrective action <br />For our records, please sign the Acknowledgement of Receipt below. <br />Send a copy of the signed letter and future information to my attention at <br />the above address. <br />
