Contact the fire department and the San Joaquin Valley Air Pollution Control District for <br /> applicable permits. <br /> 2. At least one person who holds the International Code Council (ICC) UST Installer/Retrofitter <br /> certification must be onsite when work is performed. <br /> 3. Any changes shall be submitted to EHD for approval prior to commencing work. All new <br /> submittals supercede the previous ones. <br /> 4. All piping, fittings, sumps, under-dispenser containment and tanks must be certified by an <br /> independent testing organization and labeled as required by the organization. (UL, ULC, ANSI, <br /> API, ASTM ect...See Title 23 Chapter 16 Table B for complete list) <br /> 5. All monitoring systems shall be listed on the LG113-16. <br /> 6. All testing shall be in compliance with manufacturer's instructions, section 2635 and section 2636 <br /> of California Code of Regulations (CCR) <br /> 7. All piping must slope to a containment sump with an audible/visual alarm. <br /> 8. All connections (shreader valves) from the vacuum on the piping to the vacuum source must be <br /> turned down in the piping sump. <br /> 9. All tank top openings, that are connected to the primary space of the UST must have secondary <br /> containment, audible/visual alarm and be vapor tight or terminate at grade. <br /> 10. Sumps and under dispenser containment must be constructed in a manner to prevent vapors from <br /> entering the backfill. <br /> 11. All secondary containment must be constructed to prevent water intrusion. <br /> 12. The secondary containment system must be continuously monitored as required under California <br /> Health& Safety Code (HSC) 25290.1 <br /> 13. Overfill prevention shall satisfy CCR 2635(b)(2)(B). <br /> 14. Underground storage tanks are to be anchored per the permit application. <br /> 15. The tank owner must submit the owner statement and name/ICC certification information for this <br /> facility's, "California UST System Operator"(Designated Operator)prior to the final inspection. <br /> 16. A "Certificate of Compliance for Underground Storage Tank Installation Form C" shall be <br /> submitted by the owner or the owners' agent [pursuant to California Code of Regulation (CCR) <br /> 2635(d)] before this upgrade will be considered complete by SJCEHD. <br /> Page 1 of 2 <br />
