i • <br /> The following is an itemized list of underground storage tank violations that have not <br /> been addressed for Country Club Valero as of c 17. <br /> �" P. <br /> Open violations from January 23, 2017 inspections <br /> Violation #112-Failed to test spill containers annually. <br /> Annual spill container testing was last performed on 1-29-2016 and was not tested as required at the time of <br /> inspection. As such, this testing has not been completed and is past due. This test is required once every 12 <br /> months. Immediately schedule this test and provide 48 hours notification to the EHD. Please be aware that a follow <br /> up inspection will be charged at our current hourly rate. <br /> Violation #201 - Failed to maintain alarm logs and/or records of follow up actions. <br /> Maintenance and monitoring records for the last three years were not found on site. The 7-20-2016 designated <br /> operator(DO) report listed a maintenance record for the L14 vent box alarm, however, no maintenance record was <br /> available for review at the time of inspection. These records shall be maintained on site for at least three years. <br /> Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and <br /> maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the <br /> field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; <br /> (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and <br /> monitoring records for the last three years on site and submit copies of missing records to the EHD. <br /> Violation #209 -Designated operator did not inspect all required items and/or the inspection reports not <br /> completed. <br /> The following was noted at the time of inspection: <br /> 1. The designated operator (DO) failed to check and note the correct test dates for the spill container and leak <br /> detector testing on the 2-26-2017, 3-14-2016, and 4-27-2016 designated operator monthly inspection reports. Test <br /> dates were listed as 1-14-2015 instead of 1-29-2016. <br /> 2. On the 7-20-2016, 6-27-2016, and 5-25-2016 DO inspection reports, the L14 alarm was listed with no indication <br /> when this alarm had occurred or if it was corrected. <br /> 3. On the 2-26-2016 DO inspection report, the S1 alarm(which occurred on 2-2-2016 and 2-7-2016)was not <br /> documented. <br /> 4. On the 12-28-16 DO inspection report, the S-3 alarm was identified, however, no date when this alarm occurred <br /> was provided. <br /> 5. On the 7-20-2016 DO inspection report, the L-19 alarm (which occurred on 7-18-2016)was not documented. <br /> During the monthly inspection, the designated operator shall check that all required testing and maintenance for the <br /> UST system have been completed and document the dates they were done. Ensure that designated operators <br /> performing monthly inspections at this facility are including all of the required information on the reports. <br /> Violation #305-VPH monitoring of the interstitial spaces of the UST system is not maintained. <br /> The following was noted at the time of inspection: <br /> 1)The communication(of the interstitial space and its sensor) inside the UDC 1/2 and UDC 3/4 failed when tested, <br /> indicating that no continuous communication exists between the entire interstitial space and the existing brine <br /> sensor. <br /> 2) The communication inside the 87-octane diesel sump could not be tested due to a missing brine test port. <br /> The underground tank system shall be designed and constructed with a continuous monitoring system capable of <br /> detecting the entry of the liquid-or vapor-phase of the hazardous substance stored in the primary containment into <br /> the secondary containment and capable of detecting water intrusion into the secondary containment. Also, the <br /> sensors monitoring the liquid level in the interstitial space shall be maintained so that a breach in the primary or <br /> secondary containment is detected before the hazardous substance is released into the environment. Correct <br /> immediately by having a properly licensed, trained, and certified contractor investigate and correct the failed <br /> component with a functional component, obtain a permit from the EHD, as needed. Immediately correct and <br /> submit proof of correction to the EHD. <br /> Page 1 of 2 <br />
