Financial responsibility documents have not been submitted to the San Joaquin County <br />Environmental Health Department (EHD) since 1-1-12. Current financial responsibility documents are <br />required to be submitted annually. Complete and submit a copy of the Certification of Financial <br />Responsibility and CFO letter (or equivalent) to the EHD by March 15, 2013. <br />106. The UST monitoring plan on site is not current and not approved by the EHD. The monitoring <br />plan must be current and approved by the EHD. Complete and submit a copy of the monitoring plan for <br />approval by the EHD by March 15, 2013. An approved copy will be returned for your records. <br />107. The UST site plan does not include all of the L and S liquid and vacuum sensors. List all of the <br />sensors and the zones they monitor on the site plan and submit a copy to the EHD by March 15, 2013. <br />Keep a copy in the facility file on site. <br />201. Alarm logs and service receipts were not found on site for the following alarms: <br />L1- 87 STP sump sensor - fuel alarm - 4-24-12 <br />L22- Dispenser 7/8 annular sensor - fuel alarm - 4-24-12 <br />The alarm log must include the date of the alarm and what action was taken to resolve the alarm. <br />Include a service receipt with a detailed explanation of the action taken to resolve the alarm. <br />202. An approved copy of the UST response plan was not found on site. A copy of this plan shall be <br />retained on site at all times. Inspector provided a copy to the operator. Violation resolved. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />T <br />
