Wilson, R.E.H.S. <br />Margaret Lagorio, R.E.H.S. <br />Robert McClellon, R.E.H.S. <br />Mark Barcellos, R.E.H.S. <br />The permit to repair/retrofit the Underground Storage Tank (UST) System at the above referenced location has been <br />approved with the following conditions: <br />Removal Conditions: <br />1. Contact the fire department for their permit requirements. <br />2. Samples must be taken and analyzed as stated on pages 4 and 5 of the permit application. <br />3. Per permit application, ARS Engineering will perform the soil sampling and Castle Analytical will perform the <br />analytical tests. <br />4. All analytical results must come from a California certified laboratory or this office will not accept <br />them. <br />Environmental Health Department (EHD) must witness the soil sampling. Contact EHD to arrange this <br />inspection appointment. <br />Contact Dina Abate EHS at (209) 468-3468 to arrange all inspections. Allow 48-hour notice for all <br />inspections. <br />Installation Conditions: <br />1. Contact the local fire department and San Joaquin Valley Air Pollution Control District for applicable permits. <br />2. A tank integrity test must be performed by a licensed tank tester and results received prior to scheduling the <br />final inspection. <br />3. A piping line tightness test must be performed by a licensed tank tester and results given to this office prior to <br />scheduling the final inspection. <br />4. If automatic pump shut down is provided it must satisfy 2636(g) of California Code of Regulations (CCR). If <br />the facility does not satisfy 2636(g) of CCR an annual line tightness test must be performed. <br />7. All piping and connections shall bear a UL label. <br />8. All piping must slope to a containment sump with an audible and visual alarm. <br />9. All metal equipment in contact with backfill must have corrosion protection. <br />10. Overfill prevention must satisfy CCR 2635(b)(2). <br />11. Contact EHD for the following inspections: <br />a. Primary piping <br />b. Secondary piping <br />c. 24-hour lake test <br />d. Final inspection <br />12. Primary and secondary piping must be tested according to manufactures installation instructions. Per <br />Testing/Monitoring Standards for APT Piping Systems, testing of primary and secondary piping must be <br />observed for 1 hour. And an increase or decrease in pressure indicates a leak. <br />13. Any changes in information or scope of work must be submitted to EHD prior to commencing work. <br />14. A final inspection must be performed prior to returning this facility to operation. <br />15. Submit an As -Built plan to EHD at the time of the final inspection. Note: A final inspection will not be <br />performed unless the as -built plan is submitted. <br />
