Fairmont Ave. Lodi, CA 95240 Revised SR0067540 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of <br /> the application for a permit to replace the electrical penetration in the diesel transition remote fill <br /> sump with a Diversified Products Manufacturing penetration repair kit MSZ BLS 1.1 on the <br /> underground storage tank (LIST) system at the above noted address. An addendum has been <br /> received to include removing a 3' x 3'section of cement in front of the diesel transitional sump to <br /> expose the electrical penetration on the outside of the sump and replace it with a new electrical <br /> penetration. Thea endum also includes removing a 3' x 3' section of cement next to the <br /> supply and return transition sump so that the electrical penetration can be replaced on the <br /> outside and inside of the sump. The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution <br /> Control District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for <br /> approval prior to commencing work. All new addenda supersede the previous <br /> ones. <br /> 3. Contact Aris Cacapit at (209) 468-3453 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). Please <br /> be aware that inspections for emergency work must be scheduled within five <br /> days of permit approval. <br /> a. Visual inspection: Monitoring system must be verified for all functions normal and <br /> that there are no brine leaks after the repair. <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all <br /> of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br /> b. A valid, current California UST Service Technician certificate issued by the <br /> International Code Council (ICC), and <br />
