If the source of the variation listed was due to a leak, the incident shall be reported to the Local <br />Implementing Agency within 24 hours and an unauthorized release report will be filed within 5 days. If the variation was a result <br />of bookkeeping and/or stick reading errors, attach copy of reconciliation sheet for the month in question, indicating the ex- <br />planation for the variation. <br />Submit report within 15 days following the end of the last month of the 12 -month time period covered by the report. Report to <br />be submitted to your local implementing agency by Certified Mail if possible.
