CA 94577-0655 <br /> RE: Plan for tank inspection at 2908 Ben Holt, Stockton. <br /> I have reviewed the above work plan and have found the following information to be lacking or <br /> needed to be further explained in order to approve this application: <br /> 1. Please provided the site EPA# <br /> 2. Information on how the LIL approval will be maintained on the existing tank if tank is to be <br /> modified in any way. <br /> 3. Copy of wet stamp from the engineer doing the inspection. <br /> 4. Copy of confined space entry training for the special inspector(engineer) <br /> 5. Complete information on qualifications of the industrial hygienist to be used for the inspection <br /> including confined space entry and any other special training. <br /> 6. Please fill out section 3 of check sheet and included a description of intended inspection <br /> perimeters as indicated in this section. Your special inspector may be able to provide this <br /> information. <br /> 7. Tank integrity test is required on completion. <br /> 8. The City of Stockton prior to beginning any work at the site must approve this plan. A copy of <br /> their approved permit required. <br /> 9. All pressure tests must be with an approved gas, please indicate the gas to be used for <br /> testing etc. <br /> 10. Please provide new 8-hour refresher training certificates and confined space entry or workers <br /> that have not expired. <br /> 11. This plan did not include line replacement. <br /> The above items must be provided prior to approval of plans for work at site.
