The 91B line test boot in the <br /> transition sump will also be replaced. The permit has been approved with the following <br /> conditions: <br /> 1. Contact the local fire department, local planning or building department and the San <br /> Joaquin Valley Air Pollution Control District for any applicable permits. <br /> 2. Any changes shall be submitted to the San Joaquin County Environmental Health <br /> Department(EHD)for approval prior to commencing work. All new addenda supercede <br /> the previous ones. <br /> 3. Contact Garrett Backus(468-2986)to arrange the following inspection(s)(a 48-hour <br /> notice is required): <br /> a. A soap test to verify the pipeline integrity. Isolate and pressurize each pipe <br /> following the manufacturer's guidelines. <br /> b. Hydrostatic test on each UDC. <br /> 4. All components must be certified by an independent testing organization and labeled as <br /> required by the organization. (UL,ULC,ANSI,API,ASTM,etc. See California Code of <br /> Regulations(CCR),Title 23 Chapter 16 Table B for complete list). The component must <br /> bear the appropriate mark as required by the independent testing organization. <br /> 5. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company), and <br /> b. All valid, current California UST certificates issued by the International Code <br /> Council(ICC),that are applicable to the work being performed, and <br /> c. A valid,current certificate of training from the manufacturer(s)of the <br /> component(s)being tested,repaired, or serviced; <br /> and <br /> Page 1 of 2 <br />
