Spill container testing done on 1-27-12 and Monitoring <br /> System Certification testing done on 1-27-12 and 2-24-12 will be used to satisfy the annual testing <br /> requirements. <br /> 106, 107, 202. The monitoring plan and map on site is not current and approved by the Environmental <br /> Health Department (EHD). The monitoring plan and map do not include the new vapor condensate trap <br /> high level liquid sensor or the annular space liquid sensor. List all liquid sensors with model number and <br /> location in the comments section of the monitoring plan or on the site map. Complete and submit a copy <br /> of the monitoring plan and map for approval to the EHD by June 17, 2012. An approved copy will be <br /> returned for your records. <br /> 108. Secondary containment testing for the old vapor condensate trap sump was scheduled to be <br /> performed on August 5, 2011 by Tank-tek and a test report has not been submitted to the EHD. This <br /> testing was part of the required triennial testing for the facility. A copy of the test report must be <br /> submitted within 30 days of the test. By June 17, 2012, submit a copy of the test report to the EHD or <br /> explain why the testing was not done. <br /> 204. The designated operator failed to check and note the correct test date for the spill container test on <br /> the May 10, 2012 designated operator monthly inspection report. The report indicates that the spill <br /> containers were tested on February 24, 2012. The spill containers were tested on January 27, 2012 and <br /> were not tested on February 24, 2012. During the monthly inspection, the designated operator shall <br /> check that all required testing and maintenance for the UST system have been completed and document <br /> the dates they were done.
