New dispensers will be installed with Bravo conversion frames. The permit has been <br /> approved with the following conditions: <br /> Conditions of approval: <br /> 1. Contact the local fire department,local planning or building department and the San <br /> Joaquin Valley Air Pollution Control District for any applicable permits. <br /> 2. Any changes shall be submitted to the San Joaquin County Environmental Health <br /> Department(EHD) for approval prior to commencing work. All new addenda supersede <br /> the previous ones. <br /> 3. Contact Garrett Backus(209-468-2986)to arrange the following inspection(s)(a 48-hour <br /> notice is required): <br /> a. Inspect the new dispenser installation and under dispenser containment. <br /> 4. A"Certification of Installation/Modification"form shall be submitted by the owner or <br /> the owners'agent [pursuant to(CCR)2635(d)] before this upgrade will be considered <br /> complete by EHD. <br /> 5. All components must be certified by an independent testing organization and labeled as <br /> required by the organization. (UL,ULC,ANSI,API,ASTM,etc. See California Code of <br /> Regulations(CCR),Title 23 Chapter 16 Table B for complete list). The component must <br /> bear the appropriate mark as required by the independent testing organization. <br /> 6. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid,current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company), and <br /> b. All valid, current California UST certificates issued by the International Code <br /> Council(ICC),that are applicable to the work being performed,and <br /> c. A valid,current certificate of training from the manufacturer(s)of the <br /> component(s)being tested,repaired,or serviced; <br /> Page 1 of 2 <br />
