Wilson, R.E.H.S. <br />Margaret Lagorio, R.E.H.S. <br />Robert McClellon, R.E.H.S. <br />Mark Barcellos, R.E.H.S. <br />Upon review of the file for 1856 Country Club Blvd it has come to my attention that this <br />facility is in violation of Title 23 California Code of Regulations 2636(f)(4) and 2630(d). <br />This facility was inspected on 2/4/03. At that time this facility was given 30 days to <br />perform testing for the primary fuel lines and the line leak detectors. This department has <br />not received the required notification for these tests. Receipt of the required tests is <br />necessary for your facility to remain in compliance with Title 23.
