You were required to submit this information by May 5, 2002. <br />Failure to correct the violations will result in continuing/additional violations. Failure to submit the completed <br />underground storage tank information by June 15, 2002 will result in a re -inspection billed at EHD's hourly rate, <br />which is currently $89.00 or further legal action. <br />The missing information include(s); <br />W.
