PUBLIC*HEALTH SECCE
SAN JOAQUIN COUNTY
ENVIRONMENTAL HEALTH DIVISION
Karen Furst, M.D., M.P.H., Health Officer
304 East Weber Avenue, Third Floor • Stockton, CA 95202
209/468-3420
January 11, 2002
Country Club Food & Fuel
1856 W. Country Club Blvd
Stockton CA 95204
Re: Underground Storage Tanks
A recent review of the underground storage tank file for the above referenced facility indicated that
this office has not received your annual monitoring certification, line leak detector, and line tightness
results (if applicable). According to CCR 2637 (b) this testing must be performed at least once every
12 months. In addition, this office must be notified of the test date and time within 48 hours of
performing the test. Furthermore, results from the testing must be sent to this office within 30 days
of the test completion. Please submit these results to this office by February 18, 2002.
If these tests have not been performed within the past 12 months then arrange and schedule the
annual monitoring certification, line leak detector, and line tightness test (if applicable) with a
certified technician by February 18, 2002.
If you have any questions regarding this matter you may contact Michelle Sterni Le at (209) 468-
9847.
Thank you,
)Wl&
Michelle Sterni Le
Registered Environmental Health Specialist
A Division of San Joaquin County Health Care Services
