Hammer Lane, Stockton CA 95209 <br /> Your permit application, SR0050025, to install a new spill container on the diesel fill riser and a <br /> new spill container on the 91 vapor riser at the above referenced location has been approved with <br /> the following conditions: <br /> 1. Contact the Fire Department and San Joaquin Valley Air Pollution Control District for <br /> applicable permits. <br /> 2. Any changes in information in the scope of work must be submitted to SJCEHD prior to <br /> commencing work. <br /> 3. At least one person who holds the International Code Council "UST <br /> Installer/Retrofitter" certification must be onsite AT ALL TIMES when work is <br /> performed. Additionally, this person must possess training from the component <br /> manufacturer and be a licensed contractor or directly employed by one. <br /> 4. Each person installing or testing any component must hold a certificate of training from <br /> the manufacturer of that component. <br /> 5. Contact Michelle Le at(209) 468-3453 to arrange the following inspections; a 48-hour <br /> notice is required for these inspections. <br /> a. The spill bucket must be tested to ensure it holds a minimum of 5-gallons and <br /> pass a one hour lake test or equivalent <br /> b. If the drop tube is changed then 95% fill shut off must be confirmed. <br /> 6. A third party tank integrity tests must be performed upon completion of work, prior to the <br /> contractor signing off of the jobsite. The test results must be submitted to SJCEHD <br /> within 7 days of completing this project. <br /> 7. Complete and submit a copy of a UST Installation Certificate of Compliance (Former <br /> Form C) for the 91 unleaded tank and the diesel tank no later than 30 days after work is <br /> completed. <br /> 8.
