Name: Address: Date: Feb 27, 2014 <br />SUMMARY OF VIOLATIONS <br />CLASS 1, CLASS II, or MINOR -Notice to Comply) <br />ROUTINE ANNUAL INSPECTION AND MONITORING SYSTEM CERTIFICATION <br />Class II violations: (108, 117) <br />108. Secondary containment testing for the STP sumps was last performed on October 13, 2010 and <br />was due by October 2013. Testing has not been completed and is four months past due. Secondary <br />containment testing is required once every 36 months. Immediately schedule this test and provide 48 <br />hours notification to the EHD. <br />108. Secondary containment testing for the piping, tank annulars, and fill sumps was performed on <br />August 30, 2013 and a test report has not been submitted to the EHD. A copy of the test report must be <br />submitted within 30 days of the test. Immediately submit a copy of the test report to the EHD. <br />117. According to maintenance records on site, the 87 tank STP relay was replaced on December 17, <br />2013, and a permit was not obtained from the EHD. Emergency repairs relating to UST system <br />monitoring and release detection may be performed by a properly licensed, trained, and certified <br />contractor pursuant to the EHD's � and a permit obtained within one business day from the EHD. <br />All other repairs must have a pen,, j-bmitted to and approved by the EHD prior to beginning work. <br />Immediately obtain a permit from the EHD for the 87 relay replacement.
