The designated operator failed to check and note the correct test dates for the monitoring system <br />certification and secondary containment test on the 2-15-12 designated operator monthly inspection <br />report. The correct monitoring system certification date for 2011 is 2-16-11. The correct secondary <br />containment test date is 10-13-10. During the monthly inspection, the designated operator shall check <br />that all required testing and maintenance for the UST system have been completed and document the <br />dates they were done. Ensure that designated operators performing monthly inspections at this facility <br />are including all of the required information on the reports. <br />305. The west Bravo box float and chain in the #3 dispenser sump failed to stop the flow of product at <br />the dispenser when tested. All monitoring equipment shall be maintained to activate an audible and <br />visual alarm or stop the flow of product at the dispenser when it detects a leak. The service technician <br />adjusted the chain and verified functionality. <br />Notes <br />1. Complete the following by March 23, 2012 and send to the San Joaquin County Environmental <br />Health Department: <br />a. The Return to Compliance Certification form. <br />b. A statement explaining how each violation was corrected. <br />c. Supporting documentation. <br />ALL END STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />
