The permit has been approved with the following conditions: <br /> 1. Contact the local fire department, and the San Joaquin Valley Air Pollution Control <br /> District for applicable permits and requirements. <br /> 2. Any changes in information or scope of work must be submitted to SJCEHD prior to <br /> commencing work. <br /> 3. At least one person who holds the International Code Council"UST <br /> Installer/Retrofitter"certification must be onsite AT ALL TIMES when work is <br /> performed. <br /> 4. A "Certificate of Compliance for Underground Storage Tank Installation Form C" <br /> shall be submitted by the owner or the owners'agent [pursuant to California Code of <br /> Regulation(CCR)2635(d)]before this upgrade will be considered complete by <br /> SJCEHD. <br /> 5. Contact Ray von Flue at(209)468-9848 to arrange the following inspection <br /> appointments; a 48-hour notice for inspection appointments is required. <br /> a. Overfill prevention: overfill prevention system in compliance with CCR 2635 <br /> (b)(2)must be verified. <br /> b. Spill Bucket: verify minimum 5-gallon capacity and conduct a 30-minute lake <br /> test in compliance with CCR 2635(b)(1). Proper drainage must also be verified. <br /> 6. Permit expires 90 days from the approval date.
