The existing dispensers will be reinstalled with no change to <br />them. Please find the following submittal requirements: <br />1. <br />Closure application <br />2. <br />Installation application <br />3. <br />Existing A and B forms <br />4. <br />New A and B forms <br />5. <br />Application fee <br />6. <br />Detail Sheets <br />The bid for this job will be awarded on August 24, 1998. Because the contractor has not been <br />selected a portion of this application will be incomplete. The contractor information, <br />monitoring and response plan and the Site Health & Safety Plan will be submitted by the <br />contractor as soon as he is selected. Due to the length of review time involved with a job of <br />this nature I am requesting your review at this time.
