The permit has been approved with the <br /> following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution <br /> Control District for any applicable permits and requirements. <br /> 2. Contact Jeffrey Wong to arrange the following inspections (a 48 hour notice <br /> is required): <br /> a. Verify the existing UDC sensor of Dispenser#1/2 is located to detect a <br /> leak at the earliest opportunity and functions normally after the <br /> replacement of the wire. Please be aware that the facility cannot operate <br /> before the EHD inspection and approval of this functionality test. <br /> 3. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all <br /> of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board(may be issued to the installation <br /> company), and <br /> b. A valid, current California UST Service Technician certificate issued by <br /> the International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s)being tested, repaired, or serviced, and <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 4. If concrete is broken near the UST system, a secondary containment test must be <br /> performed in accordance with the manufacturer's guidelines and witnessed by <br /> EHD.
