The permit is extended until January 6, 2011. The permit <br /> has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Garrett Backus at(209) 468-2986 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). <br /> a. Sensor functionality: Functionality of the monitoring system must be verified to <br /> ensure that monitoring has been restored to meet the requirements of Title 23 <br /> California Code of Regulations and the Health & Safety Code. A copy of the alarm <br /> history shall be printed prior to performing the "cold start" of the Veeder Root system <br /> and made available at the time of EHD inspection. <br /> Please be aware that the facility cannot operate the UST system prior to the EHD <br /> inspection and approval (EVR upgrade only). <br /> 4. If concrete will be broken near the UST or piping, a secondary containment test must be <br /> performed on the UST or piping once concrete is poured in accordance with the <br /> manufacturer's guidelines. <br /> 5. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> (1 D <br />
