Please <br />complete the following items: <br />1. Underground Storage Tanks — Facility Form A — Provide all current information, sign and date at <br />the bottom. <br />2. Information and Instructions for Obtaining A Permanent California ID Number: Complete the <br />application for a California Hazardous Waste Permanent ID Number and fax it to (916) 255-4703. <br />3. Certification of Financial Responsibility/Letter from Chief Financial Officer: Complete the form <br />provided and make a copy for our records. This document is required to be submitted annually. <br />4. Underground Storage Tank Monitoring and Response Plan: Complete and submit a copy. Retain <br />onsite for future reference by employees. <br />5. Hazardous Materials Management Plan: Contact the Office of Emergency Services, Mr. Dennis <br />Fields, at 468-3969 for information on updating this plan. <br />6. Report #5021: Update all information and sign and date. Bring with all other forms to our <br />accounting office to pay for transfer fees. <br />7. A signed "Designated UST Operator" must be submitted with current information. <br />8. Go to: http://www.watei•boards.ca.gov/cwphome/ust/re ulatory/index.htnll - rgs to obtain the <br />current Statutes (Underground Storage of Hazardous Substances) and UST Regulations that you <br />are required to have for your monitoring and response plan. <br />9. Also enclosed for your information is a Best Management Practice for Storm water Management <br />that you can use for your facility. <br />If you have any questions please contact me at (209) 468-9847. <br />Additionally, this facility is overdue for the annual monitoring certification, line leak detector, and <br />spill bucket test. Also the triennial secondary containment test is now due.
