Fees for the annual permit to operate the UST system have not been paid. No person may own or operate an UST unless a permit for its operation has been issued by the local agency to the owner or operator of the UST system. Immediately contact the EHD accounting office to make arrangements for payment to obtain a permit to operate a UST system. This is a Class I violation. 103 HSC 25292.2 Current financial responsibility documents not submitted. Current financial responsibility documents have not been submitted to the EHD. The Certificate of Financial Responsibility and the Chief Financial Officer document submitted into CERS were out dated. Current financial responsibility documents are required to be submitted annually. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, and upload the required documents. This is a repeat violation, Class ll. 106 CCR 2632 Monitoring and response plans not current or approved by the EHD. The monitoring plan is not current and/or not approved by the EHD. In the monitoring plan for the regular unleaded tank section 'Tank Monitoring is Performed Using the Following Methods" the"Leak Sensor Model#" is listed as 794390-407. This is not correct, the leak sensor model is 794390-409. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a repeat violation, Class ll. 110 CCR 2637(e) Failed to submit secondary containment test report within 30 days. Secondary containment testing was scheduled to be performed on April 22, 2015 and a test report has not been submitted to the CERS. A copy of the test report must be submitted within 30 days of the test. Immediately submit a copy of the test report to the EHD or schedule this test and provide 48 hours notification to the EHD. This is a repeat violation, Class Il. 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. Maintenance and monitoring records for the last three years were not found on site. The April 22, 2015 secondary containment test results were not on site. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a repeat violation, Class II.
