A D.O report states that a <br /> technician was notified for a December 17, 2013 Gross Line Fail Alarm, but there was no invoice or other paperwork <br /> indicating how the alarm was addressed. These records shall be maintained on site for at least three years. <br /> Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and <br /> maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the <br /> field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) <br /> results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring <br /> records for the last three years on site and submit copies to the EHD. This is a repeat violation, Class II. <br /> 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible <br /> on site at all times. Immediately locate and retain a copy on site. This is a minor violation. <br /> 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to document all the alarms from the attached alarm history on the August 2014 or the <br /> September 2014 designated operator monthly inspection report and failed to check that they were responded to <br /> appropriately. The missing alarms include: Q2-Q3 alarms for August 8, 2014,August 13, 2014 and August 14, 2014 . <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that <br /> each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. This is a minor violation. <br /> 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The 89 turbine sump sensor was not at the lowest point of the sump and not located to detect a leak at the earliest <br /> opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. <br /> The sensor was relocated to the lowest point of the sump by the service technician. Ensure that all monitoring <br /> equipment is maintained to detect a leak at the earliest opportunity. This is a Class II violation. <br /> 319 HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> Liquid was observed in the 91 turbine sump on the west side of the sump. If water could enter into the secondary <br /> containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately remove this <br /> liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, and manage it accordingly. <br /> Ensure that the 91 turbine sump is maintained free of liquid. This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by November 23, 2014 . <br /> Consent to perform the inspection, take photos and collect samples was given by Muhammad Arshad . This <br /> facility's designated operator is Alex Jabbari (ICC expiration date: 10-3-16 ) .
