The permit has been approved with the following conditions: <br />1. Contact the local fire department, and the San Joaquin Valley Air Pollution Control <br />District for applicable permits and requirements. <br />2. Any changes in information or scope of work must be submitted to SJCEHD prior to <br />commencing work. All new addendums supersede the previous ones. <br />3. At least one person who holds the International Code Council (ICC) "UST Service <br />Technician" certification and manufacturers' certificate of training for all underground <br />storage tank (UST) monitoring system components tested must be onsite at all times <br />when work is performed. <br />4. Each person testing or installing any UST monitoring system component must hold a <br />certificate of training from the manufacturer of that component. <br />5. Contact Toua Yang at (209) 468-3459 to arrange the following inspection appointment <br />(s); a 48-hour notice for inspection appointment (s) is required. <br />a. EHD must witness the functionality of the monitoring system to ensure that <br />monitoring has been restored to meet the requirements of California Code of <br />Regulations Title 23 and the Health & Safety Code. <br />6. Permit expires 90 days from the approval date.
