The <br />contractor states that concrete will not be broken for this repair. The permit has been approved <br />with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Garrett Backus at (209) 468-2986 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments). Please be <br />aware that inspections for emergency work must be scheduled within five days of permit <br />approval. <br />a. Corrosion protection: Verify corrosion protection for the direct bury spill bucket riser <br />pipe prior to backfilling (Only if concrete is broken). <br />b. Spill bucket test: Verify minimum 5 -gallon capacity and pass a one hour hydrostatic <br />test (or equivalent). Proper drainage must also be verified. <br />Please be aware that the facility cannot receive fuel in the 91 octane tank prior to <br />the EHD inspection and approval. <br />4. If concrete is broken near the 91 octane tank, a secondary containment test must be <br />performed on the 91 octane tank once concrete is poured in accordance with the <br />manufacturer's guidelines. <br />5.
