later that day (Attachment 3). <br />On October 25, 2005, Mr. Catanyag performed a routine inspection and witnessed the annual <br />monitoring system certification. Secondary containment testing was last performed on August 29, <br />2002 (Attachment 4) and was due by August 2005. The 87 and 91 octane leak detectors also failed. <br />An inspection report was left with Mr. Graffigna (Attachment 5). <br />On October 31, 2005, a permit application was submitted and approved to replace the 87 and 91 <br />octane leak detectors (Attachment 6). The leak detectors were replaced and tested under inspection <br />of this office on November 14, 2005, and a report was submitted to the EHD (Attachment 7). <br />On November 8, 2005, secondary containment testing was performed three months late, and the <br />diesel line and 91 octane piping sump failed (Attachment 8). A permit submitted for repairs on <br />January 3, 2006, was approved (Attachment 9). The diesel line and 91 octane piping sump were <br />retested under inspection of this office on January 16, 2006, and a report was submitted to the EHD <br />(Attachment 10). <br />On October 31, 2006, Mr. Ray von Flue, Sr. REHS, EHD, performed a routine UST inspection <br />(Attachment 11) and witnessed the annual monitoring system certification (Attachment 12). Water <br />was found in the diesel piping sump and removed by the technician on site. <br />On November 8, 2007, Mr. Garrett Backus, Sr. REHS, EHD, performed a routine UST inspection <br />(Attachment 13) and witnessed the annual monitoring system certification (Attachment 14). Two <br />shear valves on the dispensers would not trip when initially tested. They were adjusted and retested <br />on site. The monitoring system certification, leak detector testing, and spill container testing were <br />performed one month late, but were not noted on the inspection report. During the inspection, Mr. <br />Graffigna stated that this facility will be sold in December. On November 27, 2007, a service <br />request for a new owner UST consultation was submitted, though escrow had not closed on the sale <br />yet. A letter was mailed to the prospective owner (Attachment 15). <br />On January 7, 2008, Mr. von Flue received a call from Ms. Rupinder Padda, new owner, who stated <br />escrow was scheduled to close in about a week. Mr. von Flue asked her to call to schedule an <br />appointment for the UST consultation after escrow has closed. <br />On December 14, 2007, Ms. Padda paid the UST and hazardous waste facility transfer fees and <br />transferred the operating permit, which was valid through December 31, 2007 (Attachment 16). <br />On January 31, 2008, Mr. von Flue performed a UST consultation with Ms. Padda. Ms. Padda <br />completed all the ownership transfer forms and the property and tanks were transferred. <br />Attachments 17 and 18 include property owner information and tank owner information for the <br />former owner. <br />Attachments 19, 20, and 21 include property owner information and tank owner information for <br />the current owner, and the UST facility file narrative. <br />2 <br />
