Kettleman Ln., Lodi CA <br />SR0052697, Change of ownership UST consultation <br />The San Joaquin County Environmental Health Department has received the consultation request for the change of <br />ownership of the underground storage tanks (UST's) at the above-mentioned facility. <br />Please contact me at the phone number below to schedule the consultation once escrow has closed. Please bring a <br />copy of the Notice of Sale or Grant Deed as proof of the ownership change of the UST. <br />The following items must be completed and submitted within 30 days of the close of escrow [CA Health and Safety <br />Code section 25284 (b) & (c)] (All forms will be provided at the time of the consultation): <br />1. Underground Storage Tanks — Facility Form — Provide all current information, sign and date at the bottom. <br />2. Underground Storage Tanks — Tank Forms — Provide all current information, sign and date the bottom of <br />each form. <br />3. Certification of Financial Responsibility/Letter from Chief Financial Officer: Complete the form provided, <br />and make a copy for our records. This document is required to be submitted to this office annually. <br />4. Underground Storage Tank Monitoring and Response Plans: Complete and submit a copy. Retain the <br />originals onsite for future reference by employees. <br />5. Report #5021: Update all information for all programs at this site including the UST and Hazardous Waste <br />programs, sign and date. Bring with all other forms to our accounting office to pay for transfer fees and the <br />$98/hr UST Consulting fee, if necessary. <br />6. If you are the owner, you must submit a notification form identifying the "Designated UST Operator" for <br />each facility owned. A notification form is enclosed for your use. <br />7. Go to: http://www.waterboards.ca. og v/cwphome/ust/re ug latoD /index.html - rgs to obtain the current statutes <br />(Underground Storage of Hazardous Substances) and UST Regulations that you are required to have for your <br />monitoring and response plan. <br />8. Also enclosed for your information is a brochure of Best Management Practices (BMP's) for Retail Gasoline <br />Outlets that you can use as part of your BMP's for your facility. <br />9. Hazardous Materials Management Plan: Contact the Office of Emergency Services at 468-3969 for <br />information on updating this plan. <br />
