Contact Aris Cacapit (468-3453) to arrange the following inspection(s) (a 48-hour notice <br />is required): <br />a. The EHD must witness the functionality of the monitoring system to ensure that <br />monitoring has been restored to meet the requirements of California Code of <br />Regulations Title 23 and the Health & Safety Code. <br />b. If a "cold start" is conducted, then before the alarm history is cleared by the "cold <br />start", a current printout must be created and available for EHD inspection. <br />Inspection may occur during the functionality test. <br />5. If concrete is being broken near the UST, pipeline and tank integrity testing must be <br />performed by a SWRCB licensed tank tester in accordance with the manufacturer's <br />guidelines. Results must be received by the EHD within seven days of returning the <br />facility to operating condition. <br />6. An "Underground Storage Tank Certification of Installation/Modification" form (form C) <br />shall be submitted by the owner or the owners' agent [pursuant to (CCR) 2635(d)] before <br />this upgrade will be considered complete by EHD. <br />7. Permit expires 180 days from date of approval.
