RE: Flames Hart 1301 W Kettleman Ln. Lodi CA 9412 SR0056407
4. Contact Axis Cacapit (468-3453) to arrange the following inspection(s) (a 48-hour notice
is required):
a. The EHD must witness the functionality of the monitoring system to ensure that
monitoring has been restored to meet the requirements of California Code of
Regulations Title 23 and the Health & Safety Code.
b. If a "cold start" is conducted, then before the alarm history is cleared by the "cold
start", a current printout must be created and available for EHD inspection.
Inspection may occur during the functionality test.
5. If concrete is being broken near the UST, pipeline and tank integrity testing must be
performed by a SWRCB licensed tank tester in accordance with the manufacturer's
guidelines. Results must be received by the EHD within seven days of returning the
facility to operating condition.
6. An "Underground Storage Tank Certification of Installation/Modification" form (form C)
shall be submitted by the owner or the owners' agent [pursuant to (CCR) 2635(d)] before
this upgrade will be considered complete by EHD.
7. Permit expires 180 days from date of approval. If an extension to the expiration date is
required, a written request must be submitted to the EHD prior to the expiration date.
Thank you,
Aris Cacapit, EHS
Environmental Health Specialist
600 E. Main St.
Stockton, CA 95202
(209) 468-3453
acacapit@sjcehd.com
