Kettleman Ln. Lodi, Ca 95242 <br />Program: UST <br />SUMMARY OF VIOLATIONS <br />(CLASS 1, CLASS 11, or MINOR -Notice to Comply) <br />On site to conduct routine inspection and witness the annual monitoring system certification testing. <br />Violation: # 17. Repeat Violation: # 7, 11, 21. <br />#7 Maintenance records were not available for review for the following alarms: L3 disel pump pit stp <br />sump fuel alarm March 12 & 13, 2009; Feb. 4 & 5, 2010; Dec. 14 & 22, 2009. Employee Faisal Baz <br />stated that the contractor ElitelV pumps out the liquid and places it in the hazardous waste drums on <br />site. Maintenance records from the past three years must be available for review. Correct immediately <br />by retaining these records. Submit a copy of the ElitelV maintenance records of how they corrected the <br />above alarms to this office by 6-16-10. <br />#11 The Veeder Root 208 sensor in the diesel stp sump was positioned approximately halfway up the <br />saddle. UST system monitoring equipment must be properly located to detect a leak at the earliest <br />opportunity. Technician corrected this during inspection by positioning this sensor at the lowest part of <br />the sump. <br />#17 The 91 VMI 99 LD 2000 MLLD failed the 3 gal/hr test. The UST system monitoring equipment must <br />function properly to detect a leak. The technician corrected this during the inspection by adjusting the <br />MLLD once and retested it to pass the 3 gal/hr test. <br />#21 Approximately one cup of liquid was at the bottom of the diesel stp sump. Water in the secondary <br />containment must be disposed of properly. Technician Corrected on site by removing the liquid. <br />Complete and submit the return to compliance certification, corrective action statements for each <br />violation, and any other supporting documentation to this office by 7-16-10. <br />ALL EHD STAFF TIME ASSOCIATED wrrH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE (5115). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: Aris Cacapit (209) 4683453 6M C7� <br />Received By. <br />S <br />Title: <br />N <br />
