The <br />permit has been approved with the following conditions. <br />Conditions of approval: <br />1. Contact the local fire department, local planning or building department and the San <br />Joaquin Valley Air Pollution Control District for any applicable permits. <br />2. Any changes shall be submitted to the San Joaquin County Environmental Health <br />Department (EHD) for approval prior to commencing work. All new addenda supercede <br />the previous ones. <br />a. All components must be certified by an independent testing organization and <br />labeled as required by the organization. (UL, ULC, ANSI, API, ASTM, etc. See <br />California Code of Regulations (CCR), Title 23 Chapter 16 Table B for complete <br />list). The component must bear the appropriate mark as required by the <br />independent testing organization. Be sure that all equipment installed is <br />compatible. <br />3. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), and <br />b. All valid, current California UST certificates issued by the International Code <br />Council (ICC), that are applicable to the work being performed, and <br />c. A valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced; <br />d. Each person installing or testing any component must hold a certificate of <br />training from the manufacturer of that component. <br />Page 1 of 2 <br />
