Kettleman Lane Lodi, CA 95240 Program: UST <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> On site to conduct a routine inspection and witness the annual monitoring system certification testing. <br /> 6. Approved copies for page one of the monitoring plan was not found on site. Copies of these plans <br /> shall be retained on site at all times. Inspector provided the approved page one of the monitoring plan to <br /> the site manager Ricardo Rivera who then made a copy and filed it with the approved page two of the <br /> monitoring plan on site. Corrected on site. <br /> 13. The 91 bravo float and chain in the 7/8 under dispenser sump, 87 bravo float and chain in the 5/6 <br /> under dispenser sump, and 87 bravo float and chain in the 3/4 under dispenser sump failed to stop the <br /> flow of product at the dispenser when tested. All monitoring equipment shall be maintained to activate an <br /> audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. Technician <br /> corrected during the inspection by cleaning off the dirt stuck to the float and retested it pass in the <br /> presence of the inspector. Corrected on site. Repeat Violation. <br /> 23c. The designated operator Randy Kirby failed to attach the alarm history for the 1-3: super annular <br /> space sensor on the 2-17-11 designated operator monthly inspection report. During the monthly <br /> inspection, the designated operator shall review the alarm history for the previous month, check that <br /> each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators <br /> performing monthly inspections at this facility are including all of the required information on the reports <br /> including the alarm history report for monitoring system sensors. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by 3-23-11. <br /> vMz i� 17719 <br /> Note: The VMI LDT-890 leak detector tester equipment used during today's annual monitoring system <br /> certification testing did not have a current certification sticker from the manufacturer affixed to it. The <br /> testing of the leak detectors cannot be accepted until documentation from the manufacturer is received <br /> verifying that this particular VMI LDT-890 leak detector tester equipment has been certified by the <br /> manufacturer and forgot to affix the manufacturer's certification sticker. The identification number <br /> assigned to this VMI LDT-890 leak detector tester equipment is Service Station Systems asset <br /> #SSS00897. If the requested documentation cannot be provided.
