The designated operator employee training for Zulfigar Khan was last done on 4-24-12 and Jaspal Koont was last done on 11-2-12. The designated operator employee training is no longer current. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Ensure that employees have been trained by the designated operator and maintain the list on site. During the inspection, Jaspal Koont and Zulfigar Khan were trained by the facility designated operator. 311. The 87 line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting a 3-gallon per hour leak at 10 psi. The leak detector was replaced and retested during the inspection. A "UST Retrofit Verification with Inspector Already Onsite" form has been completed and provided to the operator and contractor. 313. The 87 direct-bury spill container failed when tested. All spill containers shall have a minimum capacity of five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. During the inspection, the technician tightened up the fill adaptor and retested the spill container to pass. Complete and submit a copy of the Return to Compliance Certification form with a statement of how each violation was corrected, including supporting documentation, by December 20, 2013. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
