Financial responsibility documents have not been submitted to the EHD. Current financial <br />responsibility documents are required to be submitted annually. Immediately complete and submit a <br />copy of the financial responsibility to the EHD. <br />108. Secondary containment testing was last performed on 1-16-08 and was completed on 2-11-11, one <br />month past due. Secondary containment testing is required once every 36 months. Take all necessary <br />precautions to ensure testing is performed in a timely manner. <br />108. Secondary containment testing was performed on 2-11-11 and a test report was submitted to the <br />EHD on 5-16-11, three months past due. A copy of the test report must be submitted within 30 days of <br />the test. Take all necessary precautions to ensure that a copy of the test reports are submitted to the <br />EHD within 30 days. <br />201. The maintenance record and alarm log for the diesel sump alarm that occurred on 1-10-11 was not <br />found on site. These records shall be kept on site for at least 3 years. Immediately locate and maintain <br />the maintenance record on site and submit copies to the EHD by 12-29-11. <br />204. The designated operator failed to document an alarm from the attached alarm history on the <br />2-15-11 designated operator monthly inspection report and failed to check that they were responded to <br />appropriately. The missing alarm includes the diesel sump alarm that occurred on 1-10-11. During the <br />monthly inspection, the designated operator shall review the alarm history for the previous month, check <br />that each alarm was documented and responded to appropriately, and attach a copy of the alarm history <br />with documentation taken in response to any alarms to the monthly report.
