This <br />information was required to be submitted by December 18, 2008. This information has not been received resulting <br />in a non-compliant status for this facility. <br />Failure to correct the violations win result in continuing/additional violations. <br />Note: All EHD staff time associated with failing to comply, including the issuance of this letter, is billed at the <br />current hourly rate ($105).
