RE: Lodi Food & Liquor, 1225 W. Lockeford St., Lodi CA (SR0056144)
5. Any changes in information and scope of work must be submitted to EHD for
approval prior to commencing work. All new addenda supersede the previous
ones.
6. Contact Aris Cacapit to arrange the following inspections (a 48 hour notice is
required):
a. One hour hydrostatic test (or equivalent) on the 91 direct bury spill
container after concrete is poured.
b. Verify the spill container has a minimum capacity of 5 gallons.
c. Verify corrosion protection prior to backfill and pouring concrete.
d. The test fluid should cover the top joint, and be no less than 1.5 inches
below the top of the 91 direct bury spill container, regardless of the spill
container's capacity.
e. Test operation of the drain valve of the 91 direct bury spill container.
7. Since concrete will be broken over the UST, a third party tank secondary
containment test (or equivalent) must be performed, once concrete is poured in
accordance with the manufacturer's guidelines. Results must be received by the
EHD within seven days of returning the facility to operating condition.
8. A "Certificate of Compliance for Underground Storage Tank Installation Form C"
shall be submitted by the owner or the owners' agent [pursuant to (CCR) 2635(d)]
before this repair will be considered complete by the EHD.
If you have any further questions, please contact me at (209) 468-3453, fax to (209) 468-
3433 or email to acacapit@sjcehd.com.
Thank you,
#>7 1- D7
Aris Cacapit
Environmental Health Specialist
