The <br />permit has been approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution <br />Control District for any applicable permits and requirements. <br />2. Contact Michelle Henry (209) 953-7699 to arrange the following inspection <br />appointment (a 48-hour notice is required for inspection appointments): <br />a. If a cold start was performed while changing the 8 input module sensors, <br />EHD must witness the functionality of the entire monitoring system to <br />ensure that monitoring has been restored to meet the requirements of <br />California Code of Regulations Title 23 and the Health & Safety Code. <br />b. EHD must witness the functionality of the new 87 STP Sump sensor to <br />ensure that monitoring meets the requirements of California Code of <br />Regulations Title 23 and the Health & Safety Code. <br />3. Any changes in information and scope of work must be submitted to EHD for <br />approval prior to commencing work. All new addenda supersede the previous <br />ones. <br />4. All Components installed must be certified by an independent testing organization <br />and bear the appropriate mark as required by the independent testing organization. <br />5. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all <br />of the following: <br />
