The <br />application has been approved with the following conditions: <br />1. Contact the local fire department Local Planning or Building Department, and the <br />San Joaquin Valley Air Pollution Control District for any applicable permits and <br />requirements. <br />2. Contact Michelle Henry at (209) 953-7699 to arrange the following inspection <br />appointment (a 48-hour notice is required for inspection appointments): <br />a. Functionality test to ensure that monitoring has been restored to meet the <br />requirements of California Code of Regulations Title 23 and the Health & <br />Safety Code. <br />3. Any changes in information and scope of work must be submitted to EHD for <br />approval prior to commencing work. All new addenda supersede the previous <br />ones. <br />4. The Technician must print out a copy of the alarm history prior to replacing the <br />monitoring panel and have the copy available at the time of the EHD inspection. <br />5. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all <br />of the following: <br />a. One of several permissible valid, current contractor's licenses issued by <br />the Contractors State License Board (may be issued to the installation <br />company), and <br />b. A valid, current California UST Service Technician certificate issued by <br />the International Code Council (ICC), and <br />c. A valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced, <br />and <br />
