The following deficiencies/discrepancies must be corrected prior to approval: <br />1. Complete permit application iters 5 —10 (attached). <br />2. Provide sampling fine information and complete the Authorization to Release form <br />(attached) for under dispenser containment samples and possible piping sample: <br />I Provide information on proposed fill sump and under dispenser containment whether is <br />UL or third party approved to be product tight and is constructed to prevent structure <br />weakening as a result of contact with any hazardous substances released from primary <br />containment. <br />4. Provide information from tank manufacture stating the compatibility of proposed scope <br />of work. <br />5.
