Current financial responsibility documents have not been submitted to the EHD. The most recent <br /> copy submitted to EHD was dated 11-14-12. Current financial responsibility documents are required to <br /> be submitted annually. Immediately submit a copy of the financial responsibility documents to the EHD. <br /> Alternatively, log into CERS, upload the required information, and submit for review by the EHD. <br /> Complete by 5-29-14. <br /> 109. Annual monitoring system certification, leak detector testing, and spill container testing was <br /> performed on 4-30-13 and a test report has not been submitted to the EHD until 6-24-13. A copy of the <br /> test report must be submitted within 30 days of the tests. Continue to ensure that a copy of test reports <br /> are submitted to the EHD within 30 days of the tests. <br /> 201. The 4-30-13 Monitoring System Certification test results were not on site. These records shall be <br /> kept on site for at least 3 years. Immediately locate and maintain the test reports on site or available for <br /> review upon request. <br /> 206. The designated operator employee training for Mandeep Dua was not current. Training <br /> documentation indicates that Mandeep Dua was last trained by the facility designated operator on <br /> 4-24-13(not within the last twelve months. Mandeep Dua was working alone this morning at the store for <br /> about an hour. Mandeep Dua explained that he was not here when the facility designated operator <br /> performed the employee training on 4-22-14.
