Current financial responsibility documents have not been submitted to the EHD. Last financial <br /> responsibility document was dated 12/2/10 and was due by 12/2/11. Current financial responsibility <br /> documents are required to be submitted annually. Complete and submit a copy of the financial <br /> responsibility to the EHD within 30 days of receiving this report. <br /> 109, 110. Annual monitoring system certification and leak detector testing were last performed on 3/8/11 <br /> and was due by 3/8/12. Monitoring certification was performed on 3/30/12 but was not completed due to <br /> the monitoring panel crashing and the technician did not have current VMI testing certification to test the <br /> leak detector. These tests are required once every 12 months. Immediately reschedule these tests and <br /> provide 48 hours notification to the EHD. Please be aware that a follow up inspection will be charged at <br /> our current hourly rate. A permit will be required for the cold start. <br /> 201. Facility does not have a alarm log. Records of all alarms shall be kept on site for at least 3 years. <br /> Immediately prepare and maintain an alarm log on site and submit copies to the EHD within 30 days of <br /> receiving this report. <br /> 202. Approved copies of the monitoring and response plans were not found on site. Copies of these <br /> plans shall be retained on site at all times. Immediately locate and retain a copy on site. <br /> 303. All the monitoring equipment connected to the veeder root panel may not working properly to <br /> ensure continuous monitoring of the tank system as required by CCR 2630(d). Veeder root monitoring <br /> panel crashed when fail safe was performed. Correct immediately by having a properly licensed, <br /> trained, and certified contractor replace the failed component with a functional component (LG 113-x <br /> listed, if applicable) and obtain a permit within one business day from the EHD.
